I-777, Application for Replacement of Northern Mariana Card
Northern Mariana Cards were issued between 1988 and 1990 to those who achieved US citizenship via the Covenant to Establish a Commonwealth of the Northern Mariana Islands in Political Union with the United States (Public Law 94-241). If you have such a card and it has been lost, damaged, or stolen, you can request a replacement card by filing USCIS Form I-777 (Application for Replacement of Northern Mariana Card). Although the US no longer issues the cards, and has not since 1990, they are still a legitimate way to prove US citizenship. Therefore, if your card is damaged or lost or stolen, file USCIS Form I-777.

If you reside in Guam, make the check or money order payable to “Treasurer, Guam.” If you reside in the US Virgin Islands, make the check or money order payable to “Commissioner of Finance of the Virgin.” Otherwise, make it payable to the “U.S. Department of Homeland Security.”
