In section 212(a)(1) of the Immigration and Nationality Act, the medical grounds for inadmissibility are listed. In general, these grounds include dangerous and communicable diseases which have been deemed to be a danger to the US public. In order to show that they do not have these medical conditions, applicants filing for a green card must submit USCIS Form I-693 (Report of Medical Examination and Vaccination Record). This form must be completed by a USCIS-approved civil surgeon and once sealed by the surgeon must not be opened by the applicant. I-693 shows the results of a medical examination and any evidence of vaccination records found.
