I-693 (Report of Medical Examination and Vaccination Record)

In section 212(a)(1) of the Immigration and Nationality Act, the medical grounds for inadmissibility are listed. In general, these grounds include dangerous and communicable diseases which have been deemed to be a danger to the US public. In order to show that they do not have these medical conditions, applicants filing for a green card must submit USCIS Form I-693 (Report of Medical Examination and Vaccination Record). This form must be completed by a USCIS-approved civil surgeon and once sealed by the surgeon must not be opened by the applicant. I-693 shows the results of a medical examination and any evidence of vaccination records found.

Number of pages of the form: 13 (form is 6 pages and instructions are 7 pages); the form is valid for only one year so applicants must schedule their appointment close to when they will need the form, leaving enough time for blood test results and other test results to come in.
