How to File Form I-102

Form I-102, Application for Replacement/Initial Nonimmigrant Arrival-Departure Document, is used to apply for replacement of a lost or damaged Form I-94 or I-95. In some cases, non-immigrants can also file Form I-102 to get a new non-immigrant arrival-departure document.

Form I-94W, Non-Immigrant Visa Waiver Arrival-Departure Record, and Form I-95, Crewman Landing Permit, are two non-immigrant arrival-departure documents that show authorities that a person has entered the U.S. legally and can remain within the U.S. for a specific period. The documents also show when the person leaves the U.S. If these documents are lost, stolen, damaged, or inaccurate, it is important to replace them at once. Failure to do so can mean legal problems as well as a request to leave the U.S.

Steps to file form I-102

1. The applicant fills out the I-102 form.

2. The applicant attaches the following required documents:

  • Copy of the I-94, if one is available. Otherwise the applicant should at least fill in the correct number of the I-94 and the details including the date of last admission and current non-immigrant status and status expiry
  • Copy of H-1B approval notice
  • Copy of employment verification letter from the employer.
  • Copy of social security number
  • Copy of valid passport

3. The applicant completes Form G-1145, E-Notification of Application/Petition Acceptance, and clips it to the front of the application, to receive an e-notification when the Form I-102 has been accepted.

4. The applicant then mails the form to the address specified in the instructions or files the form online.
