How to File Form I-90
If a U.S. permanent resident’s green card has been lost, stolen, destroyed or is about to expire he/she should file Form I-90, Application to Replace Permanent Resident Card, to have it replaced.
Steps to File Form I-90
1. The applicant completes Form I-90.
2. The applicant attaches a copy of both sides of their green card to the application, if they have it.
If the applicant is missing their green card, they must attach a letter, explaining when and possibly where they lost their green card and also attach supporting documents that prove their identity and immigration status.
3. The applicant attaches the filing fee of $450, which includes a $365 form fee and $85 biometric services fee, to the I-90 application.
If the applicant’s card was issued but never received, they are not required to pay the fee.
4. The applicant files the application at the USCIS Phoenix Lockbox facility.

5. The applicant awaits confirmation from USCIS.
If the applicant has attached a completed Form G-1145, Notification of Acceptance of Application/Petition, with the application, they will receive an email or text message when their application has been accepted. Applicants will also be issued a receipt number, which can be used to track their application. When the application is approved, the applicant will be mailed a new green card.
