How to File Form N-565
The Certificate of U.S. Citizenship is a official document which proves U.S. citizenship. If the document is lost or damaged, Form N-565, Application for Replacement Naturalization/Citizenship Document can be used to apply for a replacement. Form N-565 can also be used to apply for a new certificate if the applicant has changed their name.
Steps to File Form N-565
1. The applicant completes Form N-565 and attaches the required documents:
- Two passport-style photographs
- If the earlier certificate is damaged, then it should be included with the application.
- If applying for a new certificate because the applicant has changed their name, include their current certificate and proof of name change, such as a copy of the marriage certificate or court order showing the name change.
- A translation of all supporting documents that are not in English
2. The applicant attaches the filing fee of $345 with the application.
3. The applicant files the application at the USCIS Nebraska or Texas Service Center, depending on where they live.

4. The applicant receives the Application Receipt Notice that will have a 13-character Application Receipt number.
This can be used to track their application on the USCIS website.
5. Once the application is approved, the applicant will be mailed a replacement Certificate of Citizenship/ Naturalization.
