How to Replace Naturalization Certificate

The Certificate of U.S. Citizenship(form N-560 or N-561) is a document issued by the U.S. government as proof of U.S. citizenship. In case the certificate is lost, damaged or destroyed, you can get a replacement certificate from USCIS. Please note that in case you need to change your name on the certificate, then you need to apply for a new certificate with the new name.

Steps to Replace Naturalization Certificate

1. Should read Form N-565, Application to Replace Certificate of Naturalization/Citizenship.

2. Read the instructions carefully for filling the N-565 form.

3. Fill the application as per the instructions.

4. Attach supporting documents and photographs:

  • Two photographs as specified in the instructions;
  • If your earlier certificate is damaged, then that certificate is to be attached; and
  • If your supporting documents are not in English, then an English translation of the same to be attached.

5. Print the application and mail it along with the filing fee to the address specified in the instructions

6. Once you have mailed the package, you need to wait for the Application Receipt Notice that will have a 13-character Application Receipt Number. You can go to the USCIS web site and enter this number to follow up on your application.

7. USCIS may also request more information or evidence when you appear at their office for an interview. They may also request you to submit the originals of any documents.

8. If the interview is successful, you will then receive the replacement certificate.
