How to Request Copy of U.S. Citizenship Certificate
The Certificate of U.S. Citizenship (form N-560 or N-561) is a document issued by the U.S. government as proof of U.S. citizenship. If you have recently claimed U.S. citizenship, in most cases through parentage, you may request a Certificate of Citizenship to have a legal document showing your status.
Note that if you need to change your name on the certificate, then you need to apply for a new certificate with the new name. Only U.S. citizens can obtain a copy of their Certificates of Citizenship.
How To Request Copy of U.S. Citizenship Certificate
1. Complete USCIS Form N-600, Application for a Certificate of Citizenship.
2. Print the application and mail the application along with the filing fee (there are special cases where the fee is less – check that in the instructions), and required photographs as per the instructions to the USCIS Phoenix Lockbox facility.
3. Once the USCIS has received your application, you should receive a Certificate of Citizenship in the mail.
